Prior to your appointment:
Please review and complete the new patient forms. If you can not complete new patient forms in advance, we will provide for you. Please arrive a few minutes early to ensure you have time to complete the forms.
Please upload a copy of your insurance card and identification so that we may verify your insurance. Your appointment will be billed as a medical visit as we do not accept vision insurance at this office.
If your primary care doctor is listed on your insurance card, you may be required to have a referral. Please contact your primary care doctor to confirm whether or not a referral is required.
